The Texas Optometry Act (§351.351) requires each license holder to file with the Board a mailing address, a home address, and each business address where the licensee practices. Any changes to any of the addresses must be filed with the Board within 10 days. As of September 1, 2023, home addresses are confidential under the Texas Government Code and will not be released to the public.
Applicants and licensees must utilize the Board’s online licensing system to update their main address, business address, home address, email address, and phone number.
The Board will use your main (mailing) address for official communications from the Board. Please provide the best address so that the Board may contact you for official Board business. If you provide your home address as your mailing address, please check the box on the form indicating it is your home address. Additionally, we require a personal email address when completing the main address – do not use a generic business email.
For a business address, please provide your primary business location. This address is for inspection purposes and will be provided under a public information request. If you are not residing in Texas, but practicing virtually, you must provide an address for the primary business location in Texas where you are practicing. Do not provide an out of state business address. If you are not actively practicing in Texas but maintaining an active or inactive Texas license, please fill the Address field as “Not Currently Practicing in Texas” and fill the city, state, zip fields using your home address.
