Request to Change Status
Want to convert your license from active to inactive? Or from inactive to active?
Prior to accessing the online renewal process, you must send an e-mail to allow Board staff to configure the database to accept a change in your license status. You will receive a return e-mail notifying you when you can renew on-line.
Please include your license number and your first and last name in your e-mail request.
